Is it possible to have a complete fictional league?

I know we can play teams from all across the NFL and other league spectrums but is it possible to have a completely fictional league?

Daves is correct, however there are limitations. Go into League->League Editor and you can change the league by renaming teams, conferences and settings. You can also add new teams. However issues have been reported when doing this. Also note that changing the number of conferences / divisions is not an option.

two things. one, you can change existing teams cities/names. cities will have to be generic unless u go into the city editor and create.

or you can delete teams and add from scratch. these options are in 'League Editor'. of course you must be in owner/gm mode to do.

when I start a new game the first thing I do is change the Washington team to Warriors. now of course its Commanders but I usually go back in time.

I like to use logos/names from obscure teams. WFL teams (Portland Storm), USFL teams (Shreveport Steamers), World League, Arena League, minor leagues, and small college / high school.

there was a proposed Baltimore team called the Bombers with a logo (source of my avatar)
I like to use an old NBA Clippers logo.

a problem is that the game is set for a certain amount of teams, depending on year. sometimes it will demand wild card teams. smaller leagues 10-12 teams won't work very well(I've tried!).

you can go into Help / Open Game Folder and add / change city names / logos. 'Logos' is actually city name; there is a file there marked 'downloads' with team names/logos. (improperly named actually, should be 'cities' and then a 'logos' file).
the city plate should be size 200 x 44
team name/logo should be 125 square, but it will take up to 180 square (PNG file).

say hello to the Columbus Saurs
(the helmet/logo is from the University of Calgary, Canada)
