The "Inland Empire" east of LA & north of SD - San Bernardino & Riverside Counties combine to make the 14th largest metro area in the US.
An argument can be made for a club in NJ.
Personally I would add Charlotte & San Antonio (and if I were SA, I would be sure to go after the Hispanic market).

The ability to add a fourth (or more) division would be awesome! Also the ability to add more leagues, but this has been addressed elsewhere, and I'm sure Clay will add this feature to BM2011 (Please Clay!!!!!).
